To register your child care facility, reset your child care facility’s ASQ Account password(s), and/or update your program’s designated ASQ point of contact, please complete the form below. Providers with multiple licensed child care facilities must register each facility individually. Only 1 person can be the ASQ designated point of contact on your child care facility’s accounts. You will receive an email from de.screens@doe.k12.de.us once your request has been processed.
