All ejection and sportsmanship incidents will be reported using the forms below. This form is to be completed by the official/officials association incident coordinator following an incident or ejection. This form must be submitted to DIAA within 24 hours of completion of the contest. If you have questions regarding the reporting process, please contact Angel Prinos.
