  • Location of complaint/violation
  • Specific details of complaint/violation
  • If complaint/violation regarding a member school/student/coach/fan, has the complaint/violation been reported to the member school?  If so, when and to who.
  • If complaint/violation regarding a member official/official association, has the complaint/violation been reported to the officials association?  If so, when and to who.
