| Certification Form for Human Reader and Human Scribe Support | In order for a staff member to provide the Human Reader and Human Scribe Support on the DeSSA assessments, the school test coordinator must assure that the staff member meets the requirements to provide this support. |
| Complete Delaware Accessibility Guidelines Manual | The Accessibility Guidelines contain the accessibility features policies and procedures for the DeSSA, DeSSA-Alt, WIDA Assessments and SAT. The complete guide includes all of the appendices. |
| DeSSA Accessibility Supports and Special Requests Form | This fillable PDF is meant to assist IEP teams with determining the appropriate supports for students to participate in the general content areas statewide assessments. |
| DeSSA-Alternate Assessment Accommodation Form | This fillable form is used to record the supports for a student placed on the alternate assessment. Students must be found eligible using the Decision-Making Tool. |
| Reading and-or Scribing Access Accommodation Decision Making Tool | This tool is meant for IEP teams to assess eligibility annually for students who require a level 3 reading/scribing access accommodation on statewide assessments such as text-to-speech for ELA Passages and speech-to-text. |
