(Spanish)<\/strong><\/a><\/span><\/li>\n<li data-olk-copy-source=\"MailCompose\"><span style=\"font-size: 14pt;color: #0000ff\"><a style=\"color: #0000ff\" href=\"https:\/\/education.delaware.gov\/diaa\/wp-content\/uploads\/sites\/2\/2026\/03\/2026_27_PreparticipationPhysicalExaminationQAs.pdf\"><strong>&nbsp;Preparticipation Physical Examination (PPE) FAQs and Essential Information<\/strong><\/a><\/span><\/li>\n<\/ul>\n<div>\n<div data-olk-copy-source=\"MailCompose\"><strong>The DIAA Sports Medicine Advisory Committee (DSMAC) has approved updates to the Pre-participation Physical Examination (PPE) Medical Form for the 2026\u20132027 school year.<\/strong><\/div>\n<div data-olk-copy-source=\"MailCompose\">&nbsp;<\/div>\n<div>As the&nbsp;Delaware Interscholastic Athletic Association&nbsp;(DIAA) transitions to electronic medical consent forms and digital submission, DSMAC has implemented a few changes to streamline the medical clearance process for student-athletes participating in interscholastic athletics.<\/div>\n<div>&nbsp;<\/div>\n<div><strong>Key Updates Include:<\/strong><\/div>\n<ul>\n<li>The History Form (page 3) and the recommended Physical Examination Form (page 4) will now remain on file with the student-athlete\u2019s Primary Care Practitioner (PCP). These forms will no longer be submitted directly to DIAA or member schools.<\/li>\n<li><strong>Student-athletes will only be required to submit the updated School Athlete Medical Card (page 5)<\/strong>, which now includes a designated section for the PCP to provide clearance and participation recommendations.<\/li>\n<li>Parent\/Guardian\/Student Consent Forms (page 2) will transition to digital consent forms for Fall 2026. If your school is not using RankOne for physicals, you must submit page 2 with page 5 to your school.<\/li>\n<\/ul>\n<\/div>\n<div>\n<div>DSMAC has developed a <a href=\"https:\/\/education.delaware.gov\/diaa\/wp-content\/uploads\/sites\/2\/2026\/03\/2026_27_PreparticipationPhysicalExaminationQAs.pdf\"><strong>Preparticipation Physical Examination (PPE) FAQs and Essential Information<\/strong><\/a> resource to assist families in understanding the updated process and requirements.<\/div>\n<\/div>\n<div>&nbsp;<\/div>\n<div>For most student athletes, the history and physical forms will now stay with the PCP. The new system, RANK ONE, will allow parents\/student athletes to upload the required PPE paperwork directly. Parents\/student athletes will upload the medical card, review important medical information, and sign any consent forms directly through the website\/app.<\/li>\n<\/ul>\n<\/li>\n<li>What forms need to be submitted?\n<ul>\n<li>For the 2026-2027 academic year, the student athlete will only need to upload the medical card which now has a space for the primary care practitioner (PCP) to sign and provide clearance recommendations. As such, parents may ask their PCP for a copy of these forms to bring to the appropriate school personnel.<\/li>\n<\/ul>\n<\/li>\n<li>Who should perform the PPE?\n<ul>\n<li>The DIAA Sports Medicine Advisory Committee recommends that the required forms be completed by the student athlete\u2019s PCP to ensure continuity of medical care and coverage for services.<\/li>\n<\/ul>\n<\/li>\n<li>&nbsp;How often do I need to have the PPE?\n<ul>\n<li>All athletes should have a physical examination yearly.<\/li>\n<li>The forms must be completed after April 1st each year based on a physical examination performed by the signing physician within one year of the date of signature.<\/li>\n<li>A well-child visit or routine health check-up with your PCP usually has all of the components needed for your PCP to complete the paperwork needed for the DIAA PPE. If you are unsure, please call you PCP and ask.\n<ul>\n<li>Examples:<br \/>\u2022 If you had a routine health check up with your PCP in December, you do not need to have a new physical after April 1st. Please contact your PCP and ask them to fill out DIAA paperwork after April 1st based on the examination you had in December.<br \/>\u2022 If you had a physical examination on March 30th 2025, you would need to have a new physical examination after April 1st 2026 for the 2026-2027 school year<br \/>\u2022 Sick visits do not count as routine health examination visits and you may need a new complete physical examination if it has been greater than a year since your last PPE.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li>Does the PPE cover me during summer workouts?\n<ul>\n<li>PPE for academic year 2025-2026 will end on the first day of practice for 2026- 2027 season. Any summer activity is then considered part of academic year 2025-2026. The clearance is valid through the first day of fall practice of the following school year unless a re-examination is required.<\/p>\n<ul>\n<li>Please refer to COVID information from&nbsp;<a href=\"https:\/\/www.cdc.gov\/\" target=\"_blank\" rel=\"noopener\">Center for Disease Control and Prevention (CDC)<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/www.dhss.delaware.gov\/dhss\/dph\/index.html\" target=\"_blank\" rel=\"noopener\">Delaware Department of Public Health (DPH)<\/a>&nbsp;for latest health and safety information.<\/li>\n<li>On the history form, all questions should be answered based on complete medical history (not just in the last year).<\/li>\n<li>On the School Athlete Medical Card, <span data-olk-copy-source=\"MailCompose\">a section has been added for the PCP to provide clearance and participation recommendations.<\/span><\/li>\n<li>The date the forms are filled out does not have to be the same day that the physical was performed.
